What supporting information do I need for ADP?+
Anything that confirms your conditions or shows how they affect you: a GP or specialist letter describing what your condition stops you doing, a prescription list, a care plan or OT report, a symptom diary, and a description from someone who knows you well. Social Security Scotland says one reliable piece of formal supporting information about a condition responsible for more than a minor part of your needs can be enough.
Will Social Security Scotland gather supporting information for me?+
Yes - they have a duty to help. Give them the contact details of the people who hold relevant information and permission to approach them, and your case manager can request it directly. Bear in mind it can take longer when they gather it for you, so anything you already hold is worth sending yourself.
Can I send supporting information after applying?+
Yes. You can post or upload documents at any stage: after your application has gone in, at an award review, and with a re-determination request or appeal. Put your name on every page, send copies rather than originals, and log what you sent and when.
