Why every tick is evidence

Most symptom trackers ask how you feel. This one asks what a Social Security Scotland case manager must know: for each activity, could you do it reliably today? Each column corresponds to one of the four reliability tests in the ADP rules, and the weekly count lines up with the "majority of days" principle. When you later write your application, answer a request for information or face a review, you mine the diary for real, dated examples - "on Tuesday I could not stand long enough to cook and went without a hot meal" beats "I struggle with cooking" every time.

A diary kept day by day is contemporaneous evidence of that pattern - far stronger than trying to remember your worst days when the form or the review pack arrives. Every tick box in this diary maps to one of those legal tests.
Does the diary work for an ADP award review?+
Yes - reviews are where it shines. The review pack asks you to tick "My needs are the same" or "My needs have changed", and Social Security Scotland itself says day-to-day fluctuation does not need reporting. A diary kept in the months before your review is how you answer that tick-box honestly and confidently.
Is my information safe?+
